Are these only for pre-orders? How are they acquired?

As far as I know, yes, they were only for people who pre-ordered. I never heard of other discounts for OOTP.

When I pre-ordered, there was a link for a discount coupon code. Often when you see that somewhere else you can google for a code. I don't think that's the case here (I didn't even bother trying). I wouldn't be surprised if they don't use that at all actually. Maybe they use it for problem cases or maybe special deals that they might make with say SI staff or something. It might just be something Viatech has as an option which OOTP doesn't make use of.
Support the one-and-done DH or the SP-only DH
(I think they're both better than both the AL and NL games)
and more flexible substitution rules.

Just purchased in game. As far as I can gather, and from past experience, they don't do promo codes, but do discount for pre-orders and occasionally before the release of a new edition. (not sure on the last one, but I thought I saw that at some point for 11). I think promo codes outside that are reserved for helping review sites get the full version, see, e.g., Review: OOTP 12 | iSource
